All forms below can be filled out, then scanned and emailed to OIR (Office of International Relations): International@uafs.edu.
Application Form
- Please fill out all parts of the application and do not forget to sign and date the document.
Financial Resources Form
- If you are not sponsoring yourself, your sponsor(s) must sign this form stating they will be funding your education abroad.
Immunization Form
- It is required by State Law to have immunization records when attending a university in Arkansas.
Entrance Travel Form
This form needs to be sent to the international office once airline tickets have been purchased and the visa obtained.
Housing Application
- Please fill out this form and submit if you plan to live on campus.
CPT Form
Use this form to apply for CPT authorization. For current international students only.
