UAFS Transcript Request

Official transcripts may be requested in person, by mail, by fax, or LionsLink e-mail. You can view and print an unofficial transcript through the LionsLink.

• Come by the Records Office, upper level of the Smith-Pendergraft Campus Center, and bring a photo ID.
• Download the Request for Official Transcripts (PDF), complete, print, sign, date and return by mail or fax to the Records Office.
• Send a letter of request. Please include the following information:
  • Print your full name (including maiden or other married names on your records.)
  • Social Security Number or Student I.D. Number
  • Current Address
  • Date of Birth
  • Phone Number
  • Name and complete address where you want this transcript mailed.
  • Be sure to sign your request letter.

Please note the following regarding transcripts:

• Official transcripts cannot be released if there are any outstanding financial obligations to the University.
• If you are requesting a transcript for another education institution, accrediting board, or business, request it to be mailed directly to them. Most institutions require a transcript to be mailed to their school to be considered official.
• You may have another person pick up your transcript. That person must have from you a dated, signed, written release for identification as the party to pick up your transcript.

Request for Transfer Transcripts

Students are required to furnish official transcript(s) of grades and credits from all previous colleges/universities. Transcripts can be requested by the following options:

• For your convenience, download the Transcript Release Form (PDF), complete, print, sign, and mail to your institution(s).
• Request your institution(s) to send your official transcript(s) to UA Fort Smith.
