RSO Forms, Awards & Recognition
Registered Student Organizations are responsible for submitting required forms, maintaining compliance with university policies, and participating in opportunities that recognize outstanding leadership, service, and organizational achievement.
Required RSO Forms
To remain in good standing, organizations may be required to submit forms throughout the academic year. Forms help ensure compliance with university policies, support event planning, and promote safe and successful student engagement.
Students interested in creating a new Registered Student Organization must complete the New RSO Registration process through NUMALink and submit all required supporting documentation.
Active organizations must re-register annually through NUMALink to maintain recognized status and access university resources.
Organizations must register events through NUMALink to receive approval and ensure compliance with university policies and procedures.
Organizations participating in off-campus travel may be required to submit travel authorizations, waivers, and other supporting documentation before departure.
Organizations requesting funding or making purchases through university processes must complete all applicable financial forms and documentation.
Advisors for newly approved organizations are required to complete an Advisor Agreement Form acknowledging their role and responsibilities.

Star of Excellence Program
The Star of Excellence Program recognizes Registered Student Organizations that demonstrate exceptional involvement, leadership development, campus engagement, and organizational success throughout the academic year.
Organizations participating in the program earn recognition by completing activities and opportunities that contribute to student success and organizational growth.
Examples may include:
- Leadership development and officer training.
- Campus and community service initiatives.
- Participation in campus-wide events.
- Attendance at workshops and educational programs.
- Organizational development and planning activities.
NUMA Awards
The NUMA Awards celebrate the accomplishments of Registered Student Organizations, student leaders, advisors, and campus partners who contribute to student engagement and leadership development at UAFS.
Award nominations and applications are typically collected during the spring semester, with recipients recognized during the annual awards program.
