All participants must complete the photo release form prior to attending the workshop. This form ensures we have permission to use any photos or videos that may be taken during the event for future promotional purposes.

Being of full age, for good and sufficient consideration, the receipt and sufficiency of which is hereby acknowledged, I do hereby irrevocably consent to the recording, use, copyright, and publication by UAFS, its successors and assign, its clients and/or their advertising agencies of my voice, picture, and likeness. I understand that such recordings, likeness, and/or pictures may be used in connection with any publication, exhibit, demonstration, or product. I waive my right to inspect and/or approve the finished product, advertising copy, or recording that may be used in connection therewith and understand that UAFS may assign the rights granted to it hereunder to any other person or company. I release all claims of my own, and agree to indemnify against all claims of others arising out of the making or use of any films, tapes, photographs, or recordings of me.
